Student Recommendation Form
This recommendation form may be completed by a Liberty staff member on behalf of a student and will count as a recommendation. While official letters of recommendation are strongly encouraged, as they provide more detailed insight into the student, this form may be used as a backup option if needed (when on a time crunch).
Please note: This form is accepted only for Local Scholarship Packets and may not be used for outside scholarships that require an official letter of recommendation. Printed copies are available in the College & Career Center.
Student Form for Letter of Recommendation
This form allows students to list their extracurricular activities, achievements, and goals—often referred to as a “brag sheet.” It is a helpful resource for recommenders, as it provides valuable information to support the letter of recommendation writing process.
Please encourage students, especially seniors, to complete this form and share it with their recommenders. While many brag sheet templates exist, this is one option available to them. Printed copies are available in the College & Career Center.
